Application
To expedite your application process download and complete an application before visiting your local office. Before downloading and completing the application, make sure you have the current version of Adobe Reader installed. If the current version isn’t installed you could experience difficulties submitting the online application. To get the current version click the Adobe reader link below.
If you are offered a position, you will need to provide the following documentation:
- Completed Arizona Application or Minnesota Application
- Forms of Identification
- Current CPR and First Aid (If Applicable)
- Professional License/Certificate
- TB/Chest X-Ray (If available)
- 3 Work References (names/phone numbers)
- Driver's License/Car Insurance (If Applicable)
- Sign up and complete the online exam at the “education portal”. (MN Only)
- Fingerprint Clearance Card. (AZ Only)
After completing, print application and bring to your local office or email to careers@firstat.com. If you are unable to open the employee application follow the Adobe Reader link below to install the latest version.
